SUMMARY: The Population Health Management Department assesses the educational needs of our patient population and their families. Consistent with CQI data, the department creates and implements health intervention programs to improve the health status of the patients served by CFH. Development of training materials to improve staff health literacy and promote patient-centered health intervention activities is the responsibility of the department.

All CFH staff annual in-service training modules will be reviewed for educational content and periodic updating.

The Health Educator will be responsible for and assessing the educational needs of our patients, the target populations and their families; as well as for planning, implementing and evaluating effective methods to accommodate their needs. The position also involves developing and organizing educational materials for the clinic staff, and clients, coordinate community outreach, which includes educational seminars, workshops, and tabling activities. Additionally, this position involves traveling, the Health Educator must be willing to travel to and from various sites within four of the five boroughs as assigned by his/ her Supervisor.

Additionally, the Health Educator will analyze site data and reports in order to generate positive outcomes.

Essential Duties and responsibilities

  • Assess the educational needs of our patient population to determine appropriate programming and health interventions
  • Analyze data consistent with CQI and Quality Measures to target specific populations
  • Evaluate the needs of our patients, and implement effective measures to meet these needs such as, surveys, and PDAS
  • Promote patient centered health interventions via, workshops, and other educational activities
  • Implement educational programs to help engage the patients to participate with their own health such as, Nutrition programs
  • Develop and distribute educational materials for the clinic staff, and clients, brochures, educational handouts
  • Conduct care coordination activities and enhanced outreach which includes, phone calls, group targeted events, and one on health counseling,
  • Educate the population on HIV prevention medications such as, Prep/Pep
  • Perform HIV rapid testing, and refer clients who test positive for HIV to CFH open access health centers for confirmatory testing

Education and Experience:

  • Bachelor’s Degree required in Health Education and Health Promotion, Community Health, or Public Health
  • CHES certification preferred
  • Prior experience or training in health education or Public Health
  • Prior experience in developing, implementing and evaluating programs
  • Prior experience in creating interventions for chronic illness

Skills:

  • Strong organizational skills: able to manage diverse responsibilities and provide services at various sites.
  • Ability to establish and maintain effective working relationships with colleagues, clients and contracted providers of health care services.
  • Ability to plan, organize, implement, and evaluate programs which includes conducting needs assessments; creating action plans; and performing professional development activities.
  • Strong problem-solving skills and ability to address issues professionally and effectively.
  • Excellent oral and written communication skills needed to provide counseling to clients, document case work and provide training.

Health Educator
Community Health Action of Staten Island– Staten Island, NY 10301 (New Brighton area)

The Ryan White ESS Community Based Case Management and Health Education
Program assist Persons Living with HIV/AIDS to improve overall health and establish self-management through case management, peer navigation, and health education services. These program elements are goal-driven and focus on improving medical outcomes, achieving viral suppression, and self-management skill development.

The Health Educator provides group and individual health education sessions that focus on strategies for achieving positive health outcomes (i.e., viral suppression) independently. Sessions are curricula-based with an emphasis on basic HIV-related knowledge, treatment education, health literacy (e.g., understanding blood work results and treatment regimens, self-advocacy with medical providers), building support systems, and transmission prevention.

EXAMPLES OF DUTIES & RESPONSIBILITIES:

  • Coordinates and facilitates individual and group treatment education;
  • Conducts intakes and health assessment screenings;
  • Develop and implement health education service plans;
  • Develop health education curricula and activities;
  • Manage Health Education tools and materials including incentives and Metrocards;
  • Responsible for the timely completion of Health Education documentation, reports and documentation as assigned;
  • Recruit individuals for the intervention by conducting program outreach;
  • Assist in the oversight and delegation of tasks to the Community Health Worker;
  • Serve as coordinating member of the clients interdisciplinary team;
  • Conduct services in the field, including partnering agencies, medical facilities and at home visits when necessary;
  • Provide information about maintaining and increasing health and wellness;
  • Responsible for the recording and tracking of primary care status measures of clients;
  • Collaborate with the Case Manager in assessing client need, developing service plans, and participating in case conferences.

QUALIFICATIONS:

Associates Degree, B.A. or B.S. preferred, with 2 years of experience working in
the field of HIV/AIDS, behavioral health, substance abuse, health education, or other chronic illnesses. 1-2 years of experience facilitating health education.

SKILLS AND KNOWLEDGE REQUIRED:

  • Cultural and linguistic competence for the population served.
  • Experience and comfort with public speaking and facilitation group activities.
  • Computer literate in Word, Excel, Outlook and ability to navigate the Internet
  • Effective oral and written communication skills, including ability to listen and follow instructions; able to organize thoughts logically and explain themselves clearly; ability to manage communications tactfully and professionally with high attention to detail and accuracy.
  • Experience and proven ability to think strategically, plan effectively, and manage productively to achieve multiple high-priority goals.

Morris Heights Health Center – Bronx, NY 10453 (Morris Heights area)
  • Conducts group and individual sessions on reproductive health topics (reproduction, contraceptive methods, family planning, pregnancy, relationships, HIV, STDs, etc.)
